Your Personal Care Coordinator will meet with you personally to discuss and learn about you. We want to get to know you and grow a compassionate, professional and detailed portfolio all about you! The information you share, of course is always to your discretion and is what we use to establish a solid baseline of your current situation. In return, this information assists in formulating a plan of care for you that is rooted in your own goals of care, with immediate needs always taking precedence.
Identifying care needs alone can be overwhelming…sometimes we just don’t know where to start and that is something we can most definitely help with. Together, we will identify and prioritize your care needs so that you and your loved ones can have peace of mind that you are receiving all the relevant resources available to you to honour your goals of care. All while ensuring your safety, preserving your identity and maintaining your dignity.
Upon conclusion of our initial meeting, we will get down to work
Our relationship does not end there, we will agree upon routine, scheduled wellness checks when we will review how your current care plan is working for you. We are available to you at any time during our business hours and as life changes, we will continue to work together to amend your care plan as required so that it always reflects your unique, current situation.
Consider areas of your life that you feel challenged or stressed, we will problem solve with you and offer community resources that may help.
Consider your overall health, chronic illness or recent diagnoses, let's work together to create a detailed health history that can be used to assist you and/or your family when dealing with your care providers
Consider who your people are, who has your best interest at heart, who will speak and care for you, who are we to contact on your behalf in case of emergencies
Consider the “big picture” the topics that are often daunting to think about, for example:
#1 What are your advanced directives (ie. Direction your people can follow to honour your wishes if there ever comes a time you are unable to speak for yourself).
#2 What are your wishes and goals as life changes; do you prefer to stay in your own home with all necessary services brought in to keep you safe and happy or will you be downsizing and moving to a retirement or nursing home?
#3 Do you have arrangements made for end of life? It’s hard to think and speak about such things but let’s do it together!
